The upper lip lift is a common surgical technique that reduces the space between the nose and the upper lip to accentuate the vermilion border and reveal more of the upper incisors - both features often associated with a youthful appearance. Although the lower lip lift is a less popular approach, it is a plausible procedure for a permanent enhancement of the vermilion show. In contrast, lip fillers are minimally invasive injections - typically consisting of hyaluronic acid - that enhance lip volume and shape without changing their natural anatomical position. The current case report presents the lower lip lift as a permanent alternative to fillers, resulting in a more natural and esthetically appealing outcome that remains stable over a 2-year follow-up period. In summary, a lower lip lift provides a permanent, anatomical increase in vermilion show and lip eversion by repositioning tissue, while dermal fillers offer a temporary, volume-based enhancement of vermilion show without altering lip position. The choice depends on patient anatomy, preference, goals, and tolerance for surgical intervention.
