#72 - Naval Hospital Rota Readiness Training Program

Description

When in an operational setting, medical personnel of all ratings and levels of training may be called upon to utilize skills and knowledge not typically employed within their specialization. Improving preparedness of our medical teams is a key component of the DHA priority to build a modernized, integrated and resilient healthcare delivery system. Naval Hospital Rota has developed a readiness training and enrichment program to increase the knowledge base and familiarization of hospital personnel with practices and procedures which are either outside of their normal specialty or infrequently utilized. The pilot program is focused on four areas within the Directorate of Surgical Services (DSS): airway management, surgical scrub procedures, gynecological fundamentals, and musculoskeletal assessment. Participation is targeted for both enlisted (general duty corpsmen and technicians) as well as officers. The goal of the program is to increase the rotators’ operational readiness and familiarization across a broad range of medical experiences. Each rotation is modular to account for differing starting familiarity and rotator healthcare role. While the rotations are all currently located within DSS, participation from other directorates is highly encouraged as it increases the effectiveness of the program and strengthens its underlying goal of increasing readiness across the hospital. Rotators are given pre- and post-rotation survey assessments to gauge the effectiveness of the program and their increase in confidence and preparedness. These surveys are paired with proctor-generated post-surveys to evaluate the rotators’ actual skill level with the topics and procedures covered. These surveys will be given immediately post rotation and again at a future date to track knowledge and skill retention. Each rotation will also have one or more checklists to be distributed to rotators which cover skills and procedures covered in the rotation, with the goal of providing a quick reference if/when they have need to employ the skills in an actual situation. Plans are already underway to scale the program hospital wide, adding potential rotations such as sick call screening, burn treatment and pediatric management. The final element of the readiness program is the establishment of a quarterly training day which will focus on high fidelity simulations of low volume and high-risk events and procedures. The goal is to sustain clinical skills for these types of events given our low acuity and patient volume at Naval Hospital Rota. Executed or planned topics include Jada placement, malignant hyperthermia, and cricothyrotomy.
