At White River Junction VA Medical Center during FY 2020, All Cause Readmission rate was 11.5%, which was higher than the 11% VA average for level 2 medium complexity facilities. Further drill down has identified that 68.2% of patients readmitted to General (Acute Medicine) specialty were ages 60-79 years, identifying our scoped area of concern.
