#43 - Interagency collaboration to manage correctional-setting Hansen’s disease

Hansen’s disease (HD), also known as leprosy, is a chronic infectious disease caused by the bacterium, Mycobacterium leprae. It is most common in tropical and subtropical regions but can occur anywhere in the world. HD is highly contagious but can be cured with multidrug therapy (MDT). Case Study: A 33-year-old male from a Caribbean country male was admitted to a correctional facility in late summer 2023. He had been diagnosed with Hansen’s disease in early 2022 and was started on a multidrug therapy (MDT) treatment at a different facility. The patient although asymptomatic on arrival to the current facility had previously exhibited signs and symptoms of a type 2 HD reaction in November 2022. The patient was switched to a modified MDT regimen, which included prednisone, folic acid, methotrexate, rifabutin, moxifloxacin and minocycline. The patient's symptoms improved, and has had no signs or symptoms of relapse as of 09/2022. Discussion: This case study demonstrates the importance of interagency collaboration in the management of HD in the correctional environment. It also highlights how patient advocacy is prioritized to encourage best practices and equity of care. Conclusion: Interagency collaboration is essential for the effective management of HD in the correctional environment. This case study demonstrates the importance of working collaboratively to optimize patient care and improve outcomes.
