#54 - Synopsis of the 2023 U.S. Department of Veterans Affairs and U.S. Department of Defense Clinical Practice Guideline for Management of Bipolar Disorder

Bipolar disorder (BD) is among the most serious mental health disorders and, although relatively uncommon compared with Major Depressive Disorder (MDD), BD has a sizable impact on measures of disability and years lost to disability (YLD). Measures of BD present as a burden worldwide and reveal that BD is the 16th leading cause of YLD for all ages and ranks 6th globally among younger samples (ages 10–24). Medical conditions that have been found to be associated with the onset of BD include asthma, migraines, multiple sclerosis, prior traumatic brain injury (TBI), and irritable bowel syndrome. The VA/DoD Evidence-Based Practice Work Group convened a joint VA/DoD guideline development effort that included a multidisciplinary panel of practicing clinician experts and conformed to the Institute of Medicine's tenets for trustworthy clinical practice guidelines (CPGs). The guideline panel developed key questions in collaboration with the ECRI Institute, which systematically searched and evaluated the literature from 1 January 2012 to 31 December 2021 and developed and rated recommendations by using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) system. This guideline, supported by the John Scott Hannon Veterans Health Care Improvement Act, intended to promote evidence-based management of BD, and thereby improve patient’s clinical outcomes. The CPG is designed to assist primary care and mental health clinicians and specialists in screening for BD, determination of appropriate treatment, and delivery of individualized interventions with focus on mental health stabilization. Reducing mental health morbidity is important for the health and well-being of active-duty service members, Veterans, and their families. This poster presentation summarizes key features of the CPGs: the diagnosis of BD, nonpharmacologic and pharmacologic treatment, and supportive care.
