#207 - Veterans Health Administration Clinical Resource Hub: An Innovated regional Healthcare Delivery Model

Description

In one effort to address the growing U.S. health care workforce shortage, VHA developed pilot telehealth primary care and mental health programs in the mid to late 2010s. In 2019, the VHA launched the national Clinical Resource Hub (CRH) program as a regional telemedicine initiative aimed at growing a pool of clinical services and administrative support staff to provide those services into sites experiencing staffing shortages and to augment clinical services where supply is not meeting the demand. Delivering care predominately through telehealth technologies, a CRH was implemented in each of the 18 VHA networks and is staffed with primary care providers, registered nurse care managers, clinical pharmacists, behavioral health clinicians, and other support staff. CRH services are requested from sites throughout that region such as a VHA medical center or a community based-outpatient clinic. The CRH program is governed by a multidisciplinary national advisory board that monitors organizational alignment, strategic direction, and progress. The National CRH Operational Team is responsible for implementing the national strategy and the National CRH Operational Manual as directed by the advisory board. In the first year of implementation, CRH clinicians provided care to over 90,000 Veterans through more than 244,000 encounters across 475 sites. Each VHA network has further expanded into unique priorities and needs to support the Veterans within that network. In the past 5 years, CRHs have expanded into over 30 clinical services including but not limited to cardiology, physical therapy, comprehensive pain management, orthopedics, and substance use disorder. In FY22, the CRH program served 261,000 Veterans with over 668,000 encounters, a 68% increase over FY21. In early FY23, CRH clinicians surpassed over 2M encounters with Veterans and is consistently providing services into over 95% of all VHA sites of care.
