#31 - Supporting High Reliability Organization (HRO) Implementation Through the Development and Use of a Comprehensive HRO Dashboard

Description

The Veterans Health Administration (VHA) has been a leader in the patient safety movement for more than two decades. In February 2019, VHA launched an enterprise-wide High Reliability Organization (HRO) transformation effort and made a long-term commitment to pursuing a goal of Zero Harm. VHA has made tremendous strides along its Journey to High Reliability to become the safest healthcare system for all Veterans, patients, caregivers and Survivors. As VHA Central Office (VHACO) Program Offices, Veterans Integrated Service Networks (VISNs) and VA Medical Centers (VAMCs) advance toward HRO maturity, leaders are actualizing an organization-wide commitment to Zero Harm by strengthening organizational safety cultures, featuring empowered, collaborative frontline teams supported by engaged leadership within a climate of transparency, trust and continuous improvement. To continue this progress, VHA must continually assess, plan and align its data, data management and analytics practices to help guide HRO efforts for improving patient safety. Moreover, there is a need for leaders and stakeholders to be able to track and manage HRO progress at multiple organizational levels and domains to drive informed decision-making. To this end, the VHA HRO Enterprise Dashboard was developed to better support data-driven decisions by providing a single portal to access HRO-relevant metrics in a timely manner. These organizational metrics include those focused on: Building Capacity, Zero Harm, Just Culture and Employee Health & Safety. They better enable facilities, VISNs and VHACO Program Offices to determine where to best direct focus and efforts toward meaningful changes to reduce harm and improve patient safety. In support of VHA executive leaders, VHACO Program Office staff members and VISN/facility-level stakeholders’ shared goal of weaving High Reliability within the fabric of VHA operations and culture, the VHA HRO Dashboard provides timely data updates for a standard list of organizational metrics linked to various aspects of High Reliability. The VHA HRO Dashboard is organized to enable data to be viewed at multiple organizational levels (Enterprise, VISN and facility), ensuring that the insights displayed are relevant to end users and that the dashboard can be used both as a single source of relevant HRO data updates and as a platform to complement use of other localized data, dashboards and tools applied by individual facilities. The aim of this poster is to share the approach to the development of the VHA HRO Dashboard and the framework and governance system supporting its sustainment toward attendee awareness and application of strong practices for application to their work.
