Poster will describe a project that identified US Military personnel (active duty or veterans) who received clinical care at a Penn Medicine hospital, ambulatory practice, or home care program and connected with these members by a successful communication modality that confirmed the members enrollment at the VA or assisted the member with enrollment to obtain VA benefits if eligible. The poster exhibit discusses methods and modalities to identify US Military Active Duty and Veterans within the Penn Medicine patient population. What were the differences between the success and unsuccessful means of communication with this group. The development of a Best Practice Academic Partnership Model which encompasses three Veteran Affairs Medical Centers VAMC and all the Penn Medicine Entities. Metrics include capturing total number of US Military personnel identified, number that receive and respond to the outreach and follow up messages via Cipher, number that enroll in VA sponsored and Penn Home Health services. This program demonstrated an increased number of military and veteran personnel who received care both at the VAMC and Penn Medicine by the way of a Best Practice This Innovative Academic Partnership Model was designed to provide a streamline approach in the development of a multiorganizational joint practice of clinical service which were cost and clinically effective in the delivery of care which resulted in innovative joint practice of clinical services that increased referrals and visits of the multispecialty.
