Zero Harm is the True North for the Veterans Health Administration (VHA) Journey to High Reliability. From the journey’s outset, VHA leaders recognized the need for clear and comprehensive metrics to ensure accountability and assess progress toward becoming the Safest Health Care System for All. Advancing high reliability requires both operational and cultural transformation, using metrics aligned with the analysis and strategic needs of a diverse and complex health care system. To gauge progress toward a Culture of Safety, VHA incorporated culture-specific components within its organization-wide, anonymous All Employee Survey. Additionally, VHA has included data from the Joint Patient Safety Reporting (JPSR) system, inpatient infections, and Patient Safety Indicators (PSIs) as global measures of preventable harm relevant across VHA Medical Centers and Veterans Integrated Services Networks (VISNs). These key performance indicators are visible at enterprise and facility/network organizational levels and are prominently featured on VHA’s High Reliability Organization (HRO) Dashboard–accessible to every VHA staff member–enhancing transparency, fostering a Culture of Safety, and identifying areas in need of additional support. VHA has consistently seen increasing scores in its Culture of Safety staff member survey, improvement in the JPSR system’s Close Calls to Adverse Events ratio, and decreases in overall inpatient infection rates. This poster presentation will explore how to identify cultural change and patient safety improvements and gauge progress in the pursuit of Zero Harm.
