Required:
- Title of the presentation - 30 or fewer words
- A one paragraph summary describing the presentation; 500 words or fewer
- 3-5 outcomes (Following this presentation, the participant will be able to...)
- Required Documents: Speaker CV/Resume with email and address, Continuing Education Form, Disclosure Form
