Note: AMSUS Members receive the lowest rates.  To ensure you receive the correct registration rate, please email membership@amsus.org to confirm your membership status or to become an AMSUS member and receive the best rates!!

or PAYOR DETAILS MUST BE ENTERED AT TIME OF REGISTRATION

• Tiered Registration Rates - must be paid in full by deadline or rate will increase! Payment must be received by 15 November to receive Early Bird rates. 
• If you need a 3rd party to make payment, you MUST enter the payor's email address on the next page to enable the 'Pay Later' option. Otherwise payment is due at checkout.
• Be prepared with credit card details or payor contact info as you begin  - Incomplete registrations will NOT save!!!  
• We are unable to switch out credit cards after registration is complete.  This will be considered a cancel, and subject to cancellation fees and higher registration rates.

Registration cancellations are subject to a $75 fee.
